Use the New Jersey Motor Vehicle Commission for a New Jersey driver history abstract. The MVC DO-21 form distinguishes certified request options and asks for identifying information plus an authorized purpose when the record is for someone other than yourself.

The MVC request process distinguishes driver-history products and asks for information that identifies both the record and, when applicable, the requester. That is why it is useful to decide whether you need your own history, a certified document for another organization, or a permitted request involving someone else before completing the form. The answer determines which documentation and authorization requirements matter.
Keep the motor-vehicle scope clear when you read the result. A driver-history abstract is maintained by the MVC and is designed around licensing and driving information. It should not be described as a criminal background report or a court disposition search. If a traffic or criminal court outcome is independently important, use the court system for that record rather than inferring it from the DMV document alone.
For the online request linked from MVC’s Driver History Abstract page, have your User ID number, New Jersey driver license/ID, and a credit or debit card for the $15 fee. Use the driver license number to identify a mailed record request; DO-21 specifies name, date of birth, gender and address when that number is unavailable.
The form separates subject and requester details. To order by mail, send completed DO-21, a photocopy of your New Jersey driver license/ID and a $15 check or money order payable to NJMVC to the MVC Abstract Unit, 225 East State Street, PO Box 142, Trenton, NJ 08666-0142. The current form estimates approximately three to four weeks for mail turnaround.

Decide whether an ordinary record view or certified abstract is actually required. You can also request an abstract at an MVC agency: bring your New Jersey driver license/ID and $15, and submit DO-21, completed beforehand or at the agency. MVC lists credit/debit card, cash, check or money order for this in-person fee.
A certified abstract may be required for a formal purpose even when an ordinary personal review would be sufficient for your own information. Likewise, access to another person’s driver history can depend on an authorized use. Do not remove those conditions when summarizing the result. The safest research note states which MVC product was requested, who requested it, and the purpose category used, without implying broader public access than the official process allows.
